The RN Care Manager is responsible for assessing, planning, and coordinating the care of patients from admission through discharge. This includes managing transitions of care, reducing unnecessary hospital stays, and ensuring appropriate discharge planning for high-risk patients. The RN Care Manager works closely with the interdisciplinary care team to promote quality outcomes and efficient resource use.
Key Responsibilities:The RN Care Manager is a registered nurse who helps plan a patient’s entire hospital stay—from the moment they’re admitted to the moment they go home or to another facility. Their main goal is to make sure patients get the care they need efficiently and are discharged safely with proper follow-up plans. They look out for patients at high risk of complications or readmission , and help coordinate services like home care, rehab, or follow-up appointments. They also work with insurance companies , do clinical reviews , and ensure the hospital follows all care guidelines.
This role is a mix of clinical knowledge , communication skills , and administrative work —ideal for nurses who enjoy both patient interaction and strategic coordination.
Required Qualifications:NYS Registered Nurse (RN) license – RequiredPRI Certification – Must be obtained within 3 months of hireScreen certification – Strongly preferredCare Management certification (CMC, CCM, etc.) – Preferred3 years of clinical experience in ED, Critical Care, or Med/Surg – PreferredPrior Case Management experience – PreferredFamiliarity with healthcare regulations, payers, and discharge planning programsExperience using Milliman or InterQual criteriaStrong computer and documentation skills essential
