Job Description and Requirements

- Skillfully uses HCHB software, averaging a daily caseload of 6-7 patient visits a day.
Required:•Active Arizona or Compact/Multistate LPN license •1 year of recent, full-time geriatric home health experience as an LPN (excluding PRN or Private Duty experience)•HCHB charting experience (non-negotiable) Ref ID: 449024
